I like Practice Makes Perfect: Complete Italian Grammar by Marcel Danesi. It has clear explanations.

In terms of level: according to the preface, Complete Italian Grammar is for intermediate and advanced learners. Although this McGraw-Hill book definitely isn’t geared towards total beginners, I don’t think you need to be quite an intermediate student to use it, and I would not call it advanced.

Why? Well, just as examples, in exercises towards the end, you are asked to translate into Italian things such as, “We like that book” and “We get up early every day.” That is not advanced!

One pet peeve about the Complete Italian Grammar answer key: even when all you have to do in an exercise is fill in the blanks, the book provides the whole sentences in the answer key. When you are tired and have just done 20 other grammar exercises, it is hard to find your answers plopped in the middle of sentences—without even any boldface or underscoring to help your weary eyes.
